053 – An Introduction to POLST POLST stands for Practitioners Order for Life Sustaining Treatment. It is a physician’s order that goes a long way to making sure that your loved one’s wishes are honored when critical care is needed. This form can be especially effective when emergency medical personnel are involved. An advance directive for health care (also known as a living will) can include an instruction directive (where you outline care you do or do not want) and a proxy directive (where you appoint someone to speak on your behalf). There are four main ways to build an advanced directive. They include:
- Proxy directive alone
- Instruction directive alone
- Instruction directive with a health care representative appointed – specific direction
- Instruction directive with a health care representative appointed – decision making tracks
